No. PFC Currently, the PFC doesn’t allow clinical users and health care providers clinics to share/transfer protected health information between clinics. If a survivor is being followed at a new clinic that uses the PFC, the survivor can share their information as they choose with the new clinic to re enter the data at the new home clinic . In this case, the new clinic needs to issue a new survivor activation code for the Survivor to activate their new accountvia a care summary document that they can retrieve from the Survivor Website (if they have an active account) or that can be obtained from their previous clinic.
