To get your clinic PFC access please contact our Program Manager, Susan Krause, at skrause@texaschildrens.org
Her office will send the necessary documents including the BAA, Website Access Agreements, and security documents that must be executed between your institution and Baylor College of Medicine before access can be granted.
While awaiting the execution of agreements, clinicians can utilize the Screenings Recommendation Generator to generate a Survivorship Care Plan by visiting: https://secure.passportforcare.org/en/survivor/generator/register/
