How do I get my PFC Account ?

To access your survivor account in the Passport for Care you must have received an account access code from a participating clinic.

If you are not followed by a survivorship clinic or do not know if your clinic uses the Passport for Care, please email the navigator at to determine next steps for access.

If you have received an access code, visit

Click activate your Survivorship Account on the main page, enter your activation token to continue and submit. You will be prompted to set up your username and password.

Upon activation, the survivor will receive a six-digit code that must be entered when logging in.  This code can be generated via an app on your smartphone or sent by text message.  If the survivor decides to set up two-factor authentication at this time, he or she will be prompted to choose a method of code delivery at that time.
