What is the difference between the different guideline’s formats?

 

  • Follow-Up Guidelines: It will show a complete summary of the late effects for the Survivor sorted by body system. These late effects are divided into their associated exposure, a section (score) portion, and a screening portion. At the top of the page, you may download as a PDF. 

  • Evaluations: This section will display the full evaluations for the patient with a complete report of their follow-up guidelines and screening recommendations.  Evaluations will be divided into their system, late effect, exposure, screening, and frequency. Additionally, the screening and frequency portion will include long descriptions of the patients’ history and physical. At the top of the page, you may download as a PDF or as a Word document.

  • PLE (Potential Late Effect) Patient Output: This section will open a list of potential late effects organized alphabetically by body system and is written in lay language. This document is also available in Spanish and is designed to be printed and reviewed with the Survivor at their Survivorship clinic visit.

  • Survivor Care Plan: This section will include the treatment summary and follow up care guidelines with recommendations and is recommended as the single most important document for the Survivor to share with their outside clinicians and it also may be utilized by Survivorship clinicians (with permission) to communicate information with the Survivors other providers.

  • Health Links: This will display a list of all the associated educational COG health links sorted alphabetically and in their available in languages (English, Spanish, Chinese, French).

  • Printable Guidelines for This Patient: These are patient-specific guidelines that have all applicable follow-up guidelines and include guideline references. These are divided into COG section number, Therapeutic Agents, Potential Late effects, Periodic Evaluation (including History), Health Counseling with Further Consideration (including associated health links listed and potential considerations for the patient), and the Evidence Based Scoring with the associated body system. At the top of the page, you may download as a PDF.

  • All Printable Guidelines: This will open the entire comprehensive Children’s Oncology Group (COG) Long-term Follow-up Guidelines in PDF format. This document is the complete guidelines document and is NOT patient specific.

  • Cancer Screening Guidelines: This page displays preventative screening recommendations for adult-onset cancers sorted by organ. This page is divided into the Children’s Oncology Group (COG) section number, the organ, the standard risk parameters and screening guidelines, the highest risk parameters, and screening guidelines, and lastly the health counseling and further considerations. At the top of the page, you may download as a PDF.

  • Evidence Based Scoring: This page will display the explanation of the scoring system used to base the strength of the association of the identified late effects with the specific therapeutic agent exposure.
