Activation Code does not work

My activation code does not work, what should I do? 

Your activation code will remain valid for 21 days after your activation letter is generated or until it is used. If your letter has expired or you aren’t able to activate your account using the code, please reach out to your survivorship clinic for a new activation code or assistance.
