How do I update/edit my information in my Passport for Care app?
Most information in your Passport for Care record can only be updated by your survivorship clinic. Please contact your clinic to let them know your information needs to be updated. Once your healthcare provider confirms changes, updates will be reflected in Passport for Care app.
