Managing Survivor Portal Access

In addition to the clinician-focused website, the Passport for Care provides a Survivor Portal website intended to permit survivors and their parents/guardians to independently access treatment summaries, personalized follow-up guidelines, and applicable Health Link documents at any time. Survivors and their parents or guardians must be granted access to the survivor website by an authorized clinic staff member before they can take advantage of this resource.

Managing Survivor Portal Access

Survivor Portal accounts associated with a patient record may be created and managed in the “Survivor Account” section of the patient record, accessible either via the “Survivor” link in the upper right corner of the patient management page or in the left sidebar.

 

Granting Survivor Access

If a survivor or parent/guardian account has been added to a patient’s record, it will be displayed on this page, along with its status and any recent activity. If no survivor access has been provisioned or the existing survivor account(s) has been deactivated, you will see a “Create survivor account now” button. Clicking this will generate a new survivor invitation.

 

After an account has been generated and until it is activated by the user, you will see an activation code, expiration date, and option to print activation instructions listed on the survivor account management page. The “Activation Instructions” document, available in English and Spanish, provides easy-to-follow directions for setting up a survivor account. This document should be given to the survivor or parent/guardian to allow them to use the Survivor Portal.

 

Reviewing, Updating, or Revoking a Survivor Account Activation Token

The Survivor Activation Code is a randomly generated password that is created when a survivor account is initialized and is used to securely identify and authorize the survivor when creating a Survivor Portal account. However, this code will expire three weeks after the account is generated.
