Username and password

I forgot my Passport for Care username or password. How do I get a new one? 

If you have forgotten your Passport for Care username or password, you may click on the “Forgot your Username or password” link on the login page of your app. When you click on the “Forgot your Username or password” link, you will be prompted to enter your email address or username, and a  reset email will be sent to you. Use the link in the email to create a new password.
