Features of PFC App

What are the features of Passport for Care app? 

Passport for Care provides childhood cancer patients with an easy way to access their Passport for Care record.  This includes secure access to their medical treatment data, a care summary with a list of possible late effects (which may or may not apply to the patient), a news section of interest to survivors and a chat function to communicate with their survivorship clinic for specific information.
